| Discy Business Agrees to provide the following services: | | I. Sound and Lighting Disc Jockey Service for the date and time(s) specified above. | | II. Special music requests and announcements. | | III. Clean-up of all articles and equipment belonging to Discy Business upon completion of performance. |

| Terms of Agreement: | | 1.Discy Business will select, supply and operate equipment we feel necessary to entertain at your event. | | 2. Full Payment shall be made to Discy Business in cash or check upon completion of the performance. | | 3.Discy Business will require the performance area ready at least 60 minutes prior to the scheduled performance and 30 minutes after the performance for set up and take down. The performance area is to be no less than eight feet by ten feet. Discy Business requires at least two circuits near the performance area that adhere to contemporary safety standards and supply at least 15 amps. Preparation of the performance area is the responsibility of the purchaser even if the purchaser isn’t the owner/operator of the performance area. Failure of the performance area to be ready may necessitate a delay/cancellation of the performance and Discy Business shall not be obligated to extend the performance times nor reduce the amount of Full Payment. | | 4. Purchaser shall be responsible for the safety and security of Discy Business’s equipment from Discy Business’s arrival at the performance area to Discy Business’s departure after take down. Discy Business reserves the right to end the scheduled performance due to failure of the purchaser to provide adequate safety and security. The purchaser is fully liable for events that may ensue from the failure thereof. | | 5. If the purchaser requests a cancellation of the performance after Discy Business has left for the performance, then the purchaser shall be responsible for 1/2 of the Full Payment. Any other cancellation must be agreed upon by both parties with the exception for ill health, or equipment malfunction. In the case of ill health or equipment malfunction, purchaser will be responsible for only that portion of Full Payment corresponding to the amount of performance delivered. Discy Business shall in no event be held responsible for preparatory expenses incurred by purchaser, and purchaser shall not be responsible for preparatory expenses incurred by Discy Business. | | 6. The person signing this agreement is at least 18 years old and has fully read all provisions of the agreement and can legally enter into the agreement on behalf of the purchaser. |
